Physical Demands and Work Environment: Job Purpose or Summary:
In alignment with the company mission, vision and values, the Home Care Coordinator is responsible for ensuring after-hours operations; dedicated to supporting and treasuring Care Professionals.
Essential Functions:
Foster our mission: To attract, train, retain and treasure the best care professionals
Support our Care Management department and in-home care professionals by continuing daily efforts by providing quality care and effectively communicating changes through effective follow-up
Provide consistent outreach to build strong relationships
Performs on-call services handling call volume and documenting changes
Collaborate with teammates and Care Teams to address situations
Boost employee retention with company-wide strategic initiatives
Provide meaningful, receptive, and detail-oriented experience in all interactions
Perform additional duties as required
Available to work evening shifts between 6-8 hours from 4 pm to 11 am the following day.
Knowledge and Skill Requirements:
Customer service experience preferred
Positive attitude with a team-player mindset
High School diploma or equivalent
Excellent written and verbal communication skills
PC literacy
